Emergency First Aid/Medical Treatment


Should your child suffer injury or illness whilst playing in a SGU event, it may not always be possible to contact you during the emergency. Should you not be available to guide our course of action at the time and wish the Suffolk Golf Union to provide First Aid or seek emergency treatment, please complete the authorisation below and return to the Championship Secretary.

Age of Child & Medical Conditions

My child is under 16 years of age. I authorise the Suffolk Golf Union to arrange for my child to receive essential medical treatments from a qualified medical practitioner at a hospital or other medical centre where necessary.
My child is 16 years of age or over. I acknowledge that he/she has the right to decide for him/herself on the treatment to be received or the need to attend hospital or other medical centre but in case of an emergency (e.g. where unconscious) where he/she may not be able to consent, he/she will be taken for immediate medical assistance or to the Emergency Services prior to notification of parent/carer.

Parental Details & Consent

This document MUST be with the Championship Secretary before the player will be allowed to play. Only one Child Protection Form is required per calender year. In the event of a change of circumstances or medical conditions the form can, and should be edited. It is the responsibility of the parent/guardian to inform the Suffolk Golf Union of any changes prior to each event. All information contained in this document will be treated in the Strictest of Confidence and only released to the appropriate authorities if needed.

Forms will be destroyed on 30th October each calender year.
